Several factors should be considered to evaluate the value and cost of DUS routine after EVA: the natural history of EHIT is poorly defined, the incidence seems low, and most are asymptomatic and Kabnick 2 type (projecting only slightly in the femoral vein). In addition, routine postoperative DUS detection is not recommended for procedures with higher thromboembolic complication rates, such as joint replacement or bariatric surgery. Methods: Data on the incidence of death, EHIT and deep vein thrombosis (DVT) were derived from a systematic review after radiofrequency or laser ablation of the safe vein from two sources: (1) randomized controlled trials of the EVA (N = 1482) and a series of EVA cases (N = 12,363). The number of tests required to detect an EHIT/DVT case was calculated from the incidence of EVA databases and case series; the cost of detecting a case was estimated using the Medicare global rate program for the cost of a unilateral venous DUS study. Results: This analysis included 13,845 members treated with EVA. There were no reported deaths. The incidence of venous thromboembolism detected by DUS after EVA is 0.7 per cent. The cost of unilateral DUS according to Medicare's overall reimbursement fee for office-based studies is $106.71. It is estimated that the total cost of performing DUS in this study population is at least $1,477,399, and the amount of dollars spent by venous thromboembolism detected is $14,667. Conclusions: Current Society of Vascular Surgery/American Venous The Forum ' s recommendation is to conduct the review of DUS test results after EVA within 72 hours of operation, with a weak level of recommendation (grade 2C). The current analysis shows a low incidence of EHIT/DVT with a corresponding high cost to detect each case with routine DUS detection. These data, combined with the unclear clinical meaning of the EHIT, suggest that the post-EVA universal detection policy should be revised in the near future.
